it could be coming back, however your surgeon would know for sure. For one thing gynecomastia, the enlarged portion tends to be more solid then women’s breasts… so a surgeon would easily be able to tell if it was swelling or if the problem was recurring
bottom line… you just had a semi-major surgery, relax and if you are freaked out by the way you look, try wearing a compression shirt
